User Profife in Settings#20

We need a way for a user to see and verify their personel details including:

1. Correct spelling of first and last name
2. Correct phone number where they can be reached
3. Date of birth so providers can validate user is associated with the right record.
4. Profile picture (coming later)

The user can ensure the accuracy of the information displayed in the profile and also make changes if their contact info changes.
